You can download word processing versions of TLIE forms from this page. To use these forms, open the file in your word processor, type in required information, print the form, and send it in to TLIE with appropriate signatures. You are free to use whatever fonts you wish, and to insert tables, columns or attachments where appropriate. We are not yet comfortable with a completely electronic application that could be entered online or be transmitted by email. Many of our insureds do not have signature capability on their computers yet; a significant number do not have adequate security for transmission of the sensitive information contained on these forms.
We recommend that you use the .rtf file if possible. These are formatted with page breaks between the parts of the form. You can open either the .rtf or .txt files in most word processors by opening the word processor first, and then opening the document from the menu.
We are in the process of creating a more efficient FTP link for these downloads. In the mean time, you can download these files through your browser. After the file has been downloaded to your browser, save the file to your computer as a text or rtf file and then open it in your word processor to edit. If this does not work for some reason, please contact us and we will get the form to you by email or another method of your choice.
